PURPOSE This medical policy outlines the coverage for application of skin substitute grafts/cellular and tissue-based products (CTP) for treatment of diabetic foot ulcers (DFU), venous leg ulcers (VLU), and other wounds. APPLICABILITY Using InterQual®, Devoted Health applies MAC LCD criteria to all requests within the MACʼs applicable geographic region noted in the table below.   2. For either condition, all of the following are met: a. Evidence the member has received an assessment and received all of the standard of care treatment(s) for DFU below: i. Adequate control of Type 1 or Type 2 diabetes and any exacerbating/comorbid factors ii. Documented comprehensive assessment iii. Ankle-brachial index (ABI) and diagnostic tests as indicated iv. Debridement as appropriate v. Scheduled repositioning for pressure relief vi. Mechanical offloading therapy vii. Infection prevention/control viii. Management of exudate ix. Nonsmoker or smoking cessation/counseling provided; OR b. i. Adequate control of systemic disease and any exacerbating/comorbid factors ii. Documented comprehensive assessment iii. Ankle-brachial index (ABI) and diagnostic tests as indicated iv. Duplex scan to confirm Clinical-Etiology-Anatomy-Pathophysiology (CEAP) classification v. Debridement as appropriate vi. Scheduled repositioning for pressure relief vii. Mechanical compression therapy viii. Infection prevention/control ix. Management of exudate x. Nonsmoker or smoking cessation/counseling provided; AND

  1. Evidence of either:
    a. DFU with clinical documentation of both: i. Failure to respond to at least 4 weeks of standard treatment resulting in at least one of the following:
  2. Ulcer increase in size or depth
  3. No change in baseline size or depth
  4. No sign of improvement
  5. No indication improvement is likely (e.g., granulation, epithelialization, progress towards closing) ii. Documented compliance with prescribed treatment b. VLU with clinical documentation of both: i. Failure to respond to at least 6 weeks of standard treatment including on-going compression therapy and resulting in at least one of the following:
  6. Ulcer increase in size or depth
  7. No change in baseline size or depth
  8. No sign of improvement
  9. No indication improvement is likely (e.g., granulation, epithelialization, progress towards closing) ii. Documented compliance with prescribed treatment
  10. For both DFU and VLU, evidence of the following: a. Skin substitute utilized per the intended use as approved/regulated by the FDA, and b. No contraindications to application of skin substitute graft/CTP UMGT.CL.0024 - 2026-04-22 v3.0 DEVOTED HEALTH FOR PUBLIC USE | Page 4 of 69

Requests for repeat treatment/application within the 12-week episode of care for DFU and VLU for the MACs noted in the table above must meet ALL the following criteria:

  1. Re-assessment shows documentation the previous application was successful with evidence of at least one of the following: a. Decrease in size or depth of ulcer b. No change in baseline size or depth but signs that that improvement is likely (e.g., granulation, epithelization, progress towards closing)
  2. Skin substitute utilized per the intended use as approved/regulate by the FDA; and
  3. No contraindications to application of skill substitute graft/CTP Note: When criteria in the LCD or above are met, the skin substitute grafts/cellular and tissue-based products in the table below are considered medically necessary.   • Pressure ulcers other than DFU or VLUs Note: Reimbursement for routine and non-routine medical supplies is included in the payment rates for every Medicare home health patient. Application of skin substitutes and the skin substitute are not covered separately in the home setting. A review of current clinical literature shows there is no evidence to determine these products and services are standard medical treatment.
