HIS Bundle Study

Notes: Medicare coverage will only recognize one catheterization charge when both heart catheterization and HIS Bundle Study are performed concurrently, with an additional small charge for the HIS bundle study.

Indications

(370546) Is the patient experiencing complex ongoing acute arrhythmias? 
(370547) Is the patient experiencing intermittent or permanent heart block where pacemaker implantation is being considered? 
(370548) Has the patient recently developed heart block secondary to a myocardial infarction? 

Contraindications

(370549) Will the HIS bundle study be billed separately when obtained as part of a diagnostic endocardial electrical stimulation? 
(370550) Are both heart catheterization and the HIS Bundle Study being performed at the same time without appropriate billing adjustments?

Background for this Policy

The HIS Bundle Study is a specialized type of electrocardiography requiring catheterization of the right side of the heart and is a recognized diagnostic procedure.

Medicare coverage of the procedure would be limited to selected patients: those with complex ongoing acute arrhythmias, those with intermittent or permanent heart block in whom pacemaker implantation is being considered, and those patients who have recently developed heart block secondary to a myocardial infarction. When heart catheterization and the HIS Bundle Study are performed at the same time, the program will cover only one catheterization and a small additional charge for the study.

When a HIS bundle cardiogram is obtained as part of a diagnostic endocardial electrical stimulation, no separate charge will be recognized for the HIS bundle study.
