We have an important announcement to share with you. We are excited to announce a new procedure code search tool available on the Alliance website. The tool does not require a login and allows providers to input a procedure code (CPT or HCPC) to determine if a prior authorization may be required for Alliance members to receive certain services. Instructions:

  1. Visit the Alliance website at www.alamedaalliance.org/providers/authorizations and click the hyperlink for the Procedure Code Search tool.
  2. Select the appropriate Line of Business of your patient from the drop-down menu.
  3. Enter a valid Procedure Code/Description and click Search. Prior authorization requests may be submitted in the Alliance Provider Portal or by faxing a completed Alliance Prior Authorization Form to 1.855.891.7174. For more information on how to submit a prior authorization request, please visit the Alliance website at www.alamedaalliance.org/providers/authorizations/authorization-management.
