Benefits are provided for artificial insemination which is a medical procedure in which sperm are - • Reimbursement for sperm washing or sperm isolation will be allowed when billed for a covered female member in relation to an artificial insemination procedure. • Sperm washing (58323), sperm isolation (89260, 89261), sperm count (89310, G0027), and repeat semen analysis (89320) may be separately reimbursed when performed on the same date as the artificial insemination procedure.
