Procedure / Service Description

Surgical procedures for the purpose of attempting to correct infertility will be covered except in - those instances where an elective surgical sterilization procedure is the cause of the infertility. CDPHP considers vasovasostomy and vasovasorrhaphy (55400) non-covered procedures except in cases involving unintended trauma. Approval of such cases will be determined on an individual case basis by a CDPHP medical director.
